Nutrition & Targeted Plans x3 sessions
All medical information, no matter what the scale is, must be relayed to your trainer. Any important information from doctors, specialists, councellors or those treating the participant must be shared, in confidence with your trainer.
For children under 18 an adult acting on the participants behalf must assist the trainer with the appropriate details.

Any heart condition,
Chest pain / tightness,
Diabetes,
Dizziness or fainting,
Epilepsy,
Glandular fever,
Heart murmur,
High blood pressure > 140/90,
Liver or Kidney condition,
Palpitations,
Rheumatic fever,
Stroke.
If you answer YES for any of the above, please provide details below. A clearance letter from your usual medical practitioner will also be required to accompany this document to allow participation.
SECTION B – Does the participant experience / or has ever experienced: ·
Asthma or unusual shortness of breath,
Back pain,
Muscular pain / Cramps,
Broken bone/s.

SECTION C – Does the participant have / or has had:
Any major sporting / personal injuries,
Been hospitalized within the last 2 years,
Have / had any infectious diseases,
Is the participant on any prescribed medication,
Any conditions which may limit activity / be worsened by exercise.

It is important to be transparent with each trainer so they can give you the best training they can.
Refunds are at the discresion of the trainer and I AM management.
